Admission Criteria
Patients are admitted based on:
- Residence within our service areas.
- Eligibility for Medicare, Medicaid, VA benefits, or private insurance.
- Physician/practitioner oversight with an approved plan of care.
- Medical needs that can be safely and effectively met at home.
- Patient consent and involvement in their care plan.
We welcome referrals from families, physicians, discharge planners, friends, or the patients themselves.
